Notes: This form is what needs to be completed when selling a Sponsor banner. Remember, when selling a banner for a min. of $1,500- one player receives their summer camp for free. Be sure to write “Football” on the form and in the memo section of the sponsor check. The form also needs to have the players name on it. Feel free to contact us with any questions.
