Notes: Players and Parents! It is that time of year where all students participating in a Fall sport MUST have a completed physical and parents must update all information in the My Family ID system! More information will be made available shortly! We have uploaded the Section 6 CIPPE form that MUST be completed and signed by a doctor and uploaded into the my family ID portal once it is completed! We are asking that all physicals be completed by the first week of June! PHYSICALS ARE REQUIRED! Simply print this form and have your child’s Dr. Or licensed physician complete the form. You may also chose to go to urgent care for a few and have the physical completed! The CIPPE for has to be filled out, no other form will be accepted! It is important to understand, students will not be able to participate in Summer activities without a completed and updated physical! Please stay tuned for more information about My Family ID!
