Notes: Here is the CVUSD Sports Activities form that needs to be filled out by physician and uploaded to the Home Campus clearance site. https://www.homecampus.com/login For our returning players you can log in to last year's account you will need to update for the new 2023-2024 year with your new form. For our new players you will need to create an account. Players are cleared by this OUTSIDE NPHS site so please ensure forms are completely filled out. Last year we had numerous players denied clearance for missing a parent signature, dates, physician address and phone number etc. PLEASE ENSURE FORM IS COMPLETE BEFORE UPLOADING TO PREVENT ANY CLEARANCE DELAYS. At some point during the end of May we will be teaming up with Newbury Park Exer Urgent Care in providing physicals to all of our players. This is a fundraiser run by the NPHS athletic department and proceeds benefit the athletic program. This is an optional event. You can complete your player’s physical at your doctor of choice
